A $625,000 settlement has been reached in a class action lawsuit against Hypertension-Nephrology Associates, P.C. (“Defendant” or “HNA”) arising out of a data incident HNA experienced on or about January 20, 2024.

Who Is a Class Member?

You are part of the Settlement Class if you are a resident of the United States who was sent a notice of the Data Incident indicating that your Private Information may have been impacted in the Data Incident.

Submit A Claim Form by January 20, 2026	The only way to get Settlement benefits is to submit a Valid Claim.
Opt Out of the settlement deadline passed	Get no Settlement Class Member benefits. Keep your right to file your own lawsuit against Defendant about the legal claims in this lawsuit.
Object to the settlement deadline passed	Stay in the Settlement but tell the Court why you do not agree with the Settlement. You will still be bound by the Settlement if the Court approves it.
Do Nothing No Deadline	Get no Settlement Class Member benefits. Be bound by the Settlement.
